This form is for streaming video ONLY. To reserve books, journals, or DVDs, please use the Reserves Request Form.

Each form should only include one video or film. If you have multiple requests, please submit a new form for each video or film Request. Incomplete information may result in a delay.

If you are an instructor who would like your own materials processed for streaming video through Canvas, please fill out the Streaming Video Drop-Off Form (doc) before you drop it off at the service desk of your nearest library.

Please note. We may decline to digitize a video after copyright and fair use review. At UTA Libraries, we protect intellectual property and are in copyright law compliance.
