The bibliography below offers a selection of some academic titles that contain assessment questionnaires. These are useful to measure health issues, attitudes, and emotional states. There are psychological tests, measures of emotional stability, and other personal and social measurement instruments. These full-text assessment tools can be used to understand and assess the severity of many mental and emotional disorders or other psychosocial conditions.

The UT Arlington Libraries have most of these books containing the full-text of assessment tools, tests, and assessment measures. If you do not see what you want in this short selection, then check out Helen Hough's longer list of over 100 titles. Helen Hough is the UT Arlington Libraries's Central Sciences Information Specialist, and each of her links below will take you a list of full-text assessment tools in that specific book.

Assessing Children's Well-Being: A Handbook of Measures.
Assessing Children's Well-Being: A Handbook of Measures cover image Edited by Sylvie Naar-King, Deborah A. Ellis, and Maureen A. Frey.
Mahwah, NJ: Lawrence Erlbaum Associates, 2004.
"Behavioral medicine has now matured as a field to the point where all recognize that different populations are presented with different issues. Psychological reactions and patterns affect the health and well-being of children, as well as adults, and numerous standardized instruments for the assessment of a variety of areas of children's functioning are currently available. Yet, it can be difficult for practitioners and researchers searching through general compendia of resources for child assessment--which are frequently focused on general techniques rather than specific instruments--to identify the optimal ones to meet their particular needs and to choose among them.
This practical and comprehensive reference guide is the first to sort, present, and review all the measures that can be used to evaluate the behavioral, cognitive, and emotional aspects of children's health. It organizes the measures under eight general headings, such as quality of life, adherence, pain management, and patient satisfaction. Each chapter begins with a leading authority's overview of the underlying theoretical construct and any concerns about how to measure it. Descriptions and reviews of relevant instruments follow; these include information on administration, scoring, psychometric properties, and ordering, as well as comments by the instruments' developers.
Assessing Children's Well-Being: A Handbook of Measures will be welcomed by all those professionals and scientists who seek to assess and effectively address the complex interactions between physical health and mental health in children." (publisher's description)

Assessing Older Persons: Measures, Meaning, and Practical Applications.
Assessing Older Persons: Measures, Meaning, and Practical Applications cover image Edited by Robert L. Kane and Rosalie A. Kane, with the assistance of Marilyn Eells.
Oxford, England, UK; New York, NY: Oxford University Press, 2004.
"This book describes the multidimensional assessment of older persons, offering unique insights into the state of this art. It combines comprehensive reviews of the most widely used instruments for measuring a variety of domains (such as physiological status, cognition, affect, social function, quality of life, physical environment, and caregiver burden) with an exploration of the variety of uses to which these assessments have been put. It shows how assessment is used in various aspects of practice, including comprehensive geriatric assessment, care planning, case management, and mandated assessments. It also contains a chapter on how to college data from persons who cannot communicate. The aim throughout is to combine information on specific measures with insight into how and when each measure should be used. Its message is both an urging to use assessments to enhance practice and a caution to use them wisely. The book is designed to service as both a resource for those working actively in the field and an introduction for those who are not familiar with assessment in a given area." - (publisher's description)

Fundamentals of Psychiatric Treatment Planning, 2nd edition.
Fundamentals of Psychiatric Treatment Planning cover image By James A. Kennedy.
Washington, DC: American Psychiatric Pub., 2003.
"First published in 1992, Fundamentals of Psychiatric Treatment Planning outlines an approach that quickly became the definitive standard for writing treatment plans. Developed by clinical psychiatrist James A. Kennedy, this practical, intuitive method organizes psychiatric problems into seven categories: psychological impairment, social skills, violence, activities of daily living, substance abuse, medical impairment, and ancillary impairment. Treatment plans are developed using information gathered with the Kennedy Axis V, an instrument that has proven more successful than both the Global Assessment of Functioning (GAF) scale and the Brief Psychiatric Rating Scale (BPRS) in establishing baselines and determining outcome measures." Handbook of Clinical Rating Scales and Assessment in Psychiatry and Mental Health.
Handbook of Clinical Rating Scales and Assessment in Psychiatry and Mental Health cover image Edited by Lee Baer and Mark A. Blais.
New York, NY: Humana Press, 2010.
"Psychiatric clinicians should use rating scales and questionnaires often, for they not only facilitate targeted diagnoses and treatment; they also facilitate links to empirical literature and systematize the entire process of management. In this ground-breaking text, leading researchers provide reviews of the most commonly used outcome and screening measures for the major psychiatric diagnoses and treatment scenarios. The full range of psychiatric disorders are covered in brief but thorough chapters, each of which provides a concise review of measurement issues related to the relevant condition, along with recommendations on which dimensions to measure . and when. The Handbook also includes ready-to-photocopy versions of the most popular, valid, and reliable scales and checklists, along with scoring keys and links to websites containing on-line versions. Moreover, the Handbook describes well known, structured, diagnostic interviews and the specialized training requirements for each. It also includes details of popular psychological tests (such as neuropsychological, personality, and projective tests), along with practical guidelines on when to request psychological testing, how to discuss the case with the assessment consultant and how to integrate information from the final testing report into treatment." - (publisher's description)

Handbook of Family Measurement Techniques.
Handbook of Family Measurement Techniques cover image Edited by John Touliatos, Barry F. Perlmutter, and Murray A. Straus.
Thousand Oaks, CA: Sage Publications, 2001.
"This three-volume set is intended for researchers, clinicians, and students in the family and related fields. Volume 1 includes abstracts of 976 family measurement instruments cited in the published literature 1929-1986. Volume 2 picks up where the initial volume leaves off, with abstracts of 367 instruments published 1987-1996. Abstracts in Volume 1 and 2 are preceded by overviews, written by authorities in family studies that organize and review the instruments in each chapter. Volume 3 contains 168 of the instruments abstracted in Volume 2, reproducing them in full along with their scoring instructions. Volume 3 also provides author, instrument title, and subject indexes for all three volumes. An attempt has been made to include instruments in the Handbook that are representative of all areas of family studies, that are conceptually and psychometrically sound and that have been used to evaluate diverse populations." Measuring Race and Ethnicity.
Measuring Race and Ethnicity cover image By Larry E. Davis and Rafael J. Engel; foreword by Patricia Y. Gurin.
New York, NY: Springer, 2011.
"Racial and ethnic issues stand at the core of social, political, and economic concerns in an increasingly diverse America. Accordingly, how individuals from the various ethnic groups regard themselves - and others - is a salient focus of research studies across the disciplines. This book gathers psychological measures of common phenomena such as racial identity, acculturation, and intra- and intergroup relations enabling researchers to compare concepts across groups and better evaluate differences and disparities. Researchers in psychology, social work, and public health examining cultural and race-related topics will find an immediately relevant source of valid and reliable scales in Measuring Race and Ethnicity." - (publisher's description)

Play Diagnosis and Assessment, 2nd edition.
Play Diagnosis and Assessment cover image Edited by Karen Gitlin-Weiner, Alice Sandgrund, and Charles Schaefer.
New York, NY: Wiley, ©2000.
Series: Wiley series on personality processes.
"The leading resource on identifying children's problems through play therapy-completely revised The first edition of Play Diagnosis and Assessment was the first volume of its kind to provide a comprehensive overview of the diagnosis and assessment of children through play. Over the past several years, numerous changes within the field have encouraged the development of improved techniques that surpass traditional assessment protocols and methods, such as new scales, more focused procedures, and instruments with higher levels of reliability and validity than have been previously established. Now, this classic book has been updated to address and reflect these ongoing changes. Focusing on the needs of the clinician, this new edition presents empirically tested diagnostic tools and describes improvements to existing play therapy assessment instruments, such as new testing instruments for time-limited therapy and early intervention assessment tools for young children.
The book is divided into six sections:
Developmental Play Assessments,
Diagnostic Play Assessments,
Parent-Child Interaction Play Assessments,
Family Play Assessments,
Peer Interaction Play Assessments, and
Projective Play Assessments.
The procedures described in each section target a variety of ages and populations, representing some of the most valuable, informative, and sensitive contributions to the field. Integrating formal research with clinical methods, Play Diagnosis and Assessment, Second Edition is the definitive resource for clinicians dedicated to the identification of children's problems and the best ways to intervene through the lighthearted and productive use of play therapy." - (publisher's description)

This remarkable compendium includes reviews of more than 200 instruments for measuring anxiety-related constructs in adults. These measures are summarized in 'quick view grids' which clinicians will find invaluable. Seventy-five of the most popular instruments are reprinted and a glossary of frequently used terms is provided." (publisher's description)

The Sourcebook of Nonverbal Measures: Going beyond Words.
The Sourcebook of Nonverbal Measures: Going beyond Words cover image Edited by Valerie Manusov.
Mahwah, NJ: Lawrence Erlbaum, 2005.
"The Sourcebook of Nonverbal Measures provides a comprehensive discussion of research choices for investigating nonverbal phenomena. The volume presents many of the primary means by which researchers assess nonverbal cues. Editor Valerie Manusov has collected both well-established and new measures used in researching nonverbal behaviors, illustrating the broad spectrum of measures appropriate for use in research, and providing a critical resource for future studies. With chapters written by the creators of the research measures, this volume represents work across disciplines, and provides first-hand experience and thoughtful guidance on the use of nonverbal measures. It also offers research strategies researchers can use to answer their research questions; discussions of larger research paradigms into which a measure may be placed; and analysis tools to help researchers think through the research choices available to them. With its thorough and pragmatic approach, this Sourcebook will be an invaluable resource for studying nonverbal behavior." (publisher's description)

Tools for Strengths-Based Assessment and Evaluation.
Tools for Strengths-Based Assessment and Evaluation cover image By Catherine A Simmons and Peter Lehmann.
New York, NY: Springer Pub. Co., 2012.
"Traditionally, assessment and evaluation have focused on the negative aspects or deficits of a client's presentation. Yet strengths, health, and those things that are going "right" in a person's life are key protective factors in the prevention and treatment of many mental health problems. Thus, measuring strengths is an important component of a balanced assessment and evaluation process. This is the first compendium of more than 150 valid and reliable strengths-based assessment tools that clinicians, researchers, educators, and program evaluators can use to assess a wide array of positive attributes, including well-being, mindfulness, optimism, resilience, humor, aspirations, values, sources of support, emotional intelligence, and much more. These tools provide a clear picture of an individual's strengths while being easy to complete, score, and interpret.
The scales and instruments included are consistently formatted, organized according to construct measures, and include tools for working with adults, couples, families, children, and special populations. They represent a wide range of theoretical approaches and were written by a diverse array of professionals, including social workers, psychologists, nurses, physicians, and sociologists." - (publisher's description)
