Course Reserve Lending Agreement
The UTA Libraries can loan out the resource to active UTA users. The UTA Libraries, UTA, and UT System are not responsible for any damage or loss of loaned materials. I understand that the resource(s) will be altered in the process for preparing for Libraries use (eg. Barcode added, temporary call number, magnetic strips, RFID tags). For additional information, refer to the UTA Libraries Course Reserves Policy (opens new window or tab).
If the return timeline of use is unknown, then the lender must submit a Faculty Reserve Request Form for the loaned materials, as per the Course Reserves Policy (opens new window or tab). If a request is not made for a semester, then the Course Reserve team will assume that the loaned item(s) are not in use and start the process of returning the materials.
