This form is for streaming video ONLY. To reserve books, journals, or DVDs, please use the Reserves Request Form.

Each form should only include one video or film. If you have multiple requests, please submit a new form for each video or film Request. Incomplete information may result in a delay.

If you are an instructor that wishes to lend your personal DVD copy to the library for streaming through Canvas, we will contact you for next steps.

Please note: We may decline to digitize a video after copyright and fair use review, as well as caption availability for accessibility purposes. At UTA Libraries, we protect intellectual property and are in copyright law compliance.
